Patient Registration

Before you come to New Jersey Surgery Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. A nurse from the Surgery Center will contact you to obtain your health history and give you specific details about preparing for your procedure. The following forms may be reviewed prior to your scheduled procedure. You will complete and sign them during registration at New Jersey Surgery Center on your day of surgery.
