Patient Registration

Before you come to New Jersey Surgery Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. A nurse from the Surgery Center will contact you to obtain your health history and give you specific details about preparing for your procedure. The following forms may be completed prior to your scheduled procedure.

• Patient's Bill of Rights and Responsibilities (pdf)
• Consent for Admission and Treatment (pdf)

You will need Adobe Acrobat Reader to view and print these forms.

In the event of any difficulties or complications, call your surgeon immediately.
